About the Role

The Case Manager will conduct assessments, determine eligibility for various programs, develop individualized case plans, link individuals, families with community-based housing and service resources, assist in budgeting and managing access to financial assistance funds, and provide comprehensive case management services in conjunction with community partners.

Requirements

Responsibilities:

  • Meet with clients and collect all documentation required for a thorough intake of each client in database.

  • Maintain a case load of approximately 30 clients in need of case management and supportive services to obtain or sustain housing.

  • Develop individualized service plans based on needs assessments.

  • Provide case management services including, but not limited to; providing appropriate resources, tools, and coaching to assist participants in achieving their case plan goals; making referrals for services; following up to ensure that referrals are completed; and tracking and documenting participant progress.

  • Ensure confidentiality when handling all aspects of client information.

  • Maintain up-to-date knowledge and understanding of various community resources as well as current regulations and requirements for obtaining public benefits, housing, and other services.

  • Communicate with HPC Program Director and the HPC case management team frequently to coordinate support.

  • Collaborate with other agencies and support resources to maximize participant outcomes and program goals.

*Community outreach will be required.



Qualifications:

Associates / Bachelor's degree and/or case management experience in a housing related field strongly preferred. Proficient with Microsoft applications; Word, Excel and database systems. Precise record keeping, excellent oral and written communication skills and a commitment to teamwork is essential. Must have a valid MA driver's license and provide own transportation, and must currently reside on Cape Cod.


Job Type: Full-time

Salary: $25.00 - $30.00 per hour
