About the Role
The Intake Coordinator position is the initial point of contact for HPC clients and is responsible for completing a client intake and referring them to case management staff. This position requires responding to clients contacting HPC by phone, by email and walk-ins. Qualifications for this position include the ability to accurately capture and enter client information in an electronic database, strong interpersonal and communication skills, ability to be compassionate, ability to interact with high-risk clients while being respectful and keeping appropriate boundaries. Reports to Program Director.

Responsibilities:
Conduct an initial client intake, ensuring that all client data is collected and entered into the client database in an accurate and timely manner, then refer client to HPC case manager.
Identify clients who live outside the HPC service area and make referrals to appropriate partner agencies in the area where the client lives.
Review client mail received at HPC and forward onto appropriate case manager in a timely manner, including sending a scan of materials to case managers who are working remotely; If case manager will be unavailable (I.e., is on vacation), give to Program Director.
Process direct assistance payments and provide case manager with copy of completed requisition.
Send forms and approval letters for Rental Assistance to landlords and tenants as requested by Program Director.
Assist in keeping copies of all housing applications stocked so that they are available for case managers.
As time permits, assist clients with completing housing applications selected by case manager.
Maintain professional boundaries with all current, past, and prospective clients, and maintain the confidentiality of clients and staff.
Present professional and positive image as a representative of HPC.
Perform other related duties as assigned.
