Case Manager

The Homeless Prevention Council is a nonprofit providing personalized case management services to support self-sufficiency and stability for residents of Lower Cape Cod. HPC is seeking a Case Manager to work with our dedicated team. The Case Manager will conduct assessments, determine eligibility for various programs, develop individualized case plans, link individuals, families and unaccompanied youth with community-based housing and service resources, assist in budgeting and managing access to financial assistance funds, and provide comprehensive case management services in conjunction with community partners.


Meet with clients and collect all documentation required for a thorough intake of each client.

  • Maintain a case load of approximately 30 clients in need of case management and supportive

    services to obtain or sustain housing.

  • Develop individualized service plans based on needs assessments.

  • Provide case management services including, but not limited to; providing appropriate resources,

    tools, and coaching to assist participants in achieving their case plan goals; making referrals for services; following up to ensure that referrals are completed; and tracking and documenting participant progress.

  • Ensure confidentiality when handling all aspects of client information.

  • Maintain up-to-date knowledge and understanding of various community resources as well as

    current regulations and requirements for obtaining public benefits, housing, and other services.

  • Communicate with HPC Program Director and the HPC case management team frequently to

    coordinate supports.

  • Collaborate with other agencies and support resources to maximize participant outcomes and

    program goals.


Bachelor’s degree and case management experience in a human service-related field strongly preferred. Proficient with Microsoft applications; Word, Excel and database systems. Precise record keeping, excellent oral and written communication skills and a commitment to teamwork is essential. Must have a valid MA driver's license and provide own transportation. Some community outreach will be required.

AmeriCorps VISTA Position

Are you interested in working for a growing non-profit here on Cape Cod? We help individuals and families develop self-sufficiency and stability on the Lower Cape. As a VISTA, you will support HPC's community engagement efforts and agency expansion as well develop new means to interact in the community among clients, donors and stakeholders in virtual and innovative community spaces. Duties may include:

  • Supporting implementation of annual programs

  • Providing support for special events

  • Assisting with marketing, including basic graphic design

  • Assisting with client and donor database management

  • Supporting community outreach and volunteer engagement

  • Managing social media and organization website

  • Helping to compile statistical reports and assist with grant writing as needed

This is a great experience to learn critical communication skills and support a non-profit that changes lives every day. Those with ties to the Lower Cape community are strongly encouraged to apply.

What is AmeriCorps VISTA?

AmeriCorps VISTA (or Volunteers in Service to America) was founded as the domestic version of the Peace Corps. It is a voluntary civil society program supported by the U.S. federal government that engages adults in public service with a goal of helping others and meeting critical needs in the community.

VISTAs provide one year of full-time service to their host sites in exchange for a living allowance. VISTAs serve primarily in nonprofits, community organizations, and public agencies to create and expand programs that seek to alleviate poverty.


There are currently over 5,000 VISTA members serving in 1,200 VISTA programs nationwide.
