Juventud y Apoyo para adultos jóvenes

Program Overview

Trabajando en asociación con el condado de Barnstable, HPC brinda administración de casos para jóvenes y adultos jóvenes menores de 25 años que viven solos y tienen dificultades para encontrar o mantener una vivienda. Si es un adulto joven que necesita apoyo, llame o envíe un mensaje de texto a Mary, nuestra administradora de casos de jóvenes y adultos jóvenes, al 774-801-9502.

 

El programa se centra en adultos jóvenes que corren el riesgo de quedarse sin hogar, incluidos jóvenes que hacen surf en sofás, acampan o viven en automóviles.

The range of services provided includes:

Community Outreach

The Outreach team will be available to connect with young people at various locations, including but not limited to schools, community centers, neighborhoods, partner agency sites and local community events.

Basic

Needs Assistance

Outreach workers will strive to meet your basic needs, including food, clothing, hygiene products, transportation, and more.

Referral Services

Referrals for essential resources such as housing, healthcare, education, employment, parenting support, and more will be provided, as needed. 

1:1 Support

Our team is trained to provide individualized support to meet client needs. Outreach workers are committed to remaining flexible and responsive while working with a young adult to achieve stability.

Meet Outreach Worker Amanda

Originally from Sandwich, MA., I have always known the Cape to be home. I am adamant in the belief that young adults on the Cape and Islands deserve to be able to live comfortably and securely without having to relocate. I support all young adults in our community- together we bring much needed value, innovation, and diversity to the Cape and Islands. 

What part of the Cape did you grow up in?

Sandwich

What is your favorite childhood memory of growing up on Cape?

I was raised by a single father who is my best friend. When I was a kid my dad would take me to the Deli in Sandwich for lunch, we’d go to the canal to eat our sandwiches and watch the fisherman and boats pass by. To this day, I still get the same order from the Deli: turkey sandwich on a bulky roll, a bottle of fresca, and a bag of salt and vinegar Cape Cod potato chips.  

 

What are some activities you enjoy going to in your community?

Some of my favorite activities include exploring the White Cedar Swamp trail in Wellfleet, catching a Friday Night Evening Lecture in the Marine Biological Laboratory’s Lilly Auditorium, and in the wintertime stopping by to see the Gibb’s Family Christmas light display in Sandwich.  

 

Meet Outreach Worker Alex 

The future of Cape Cod is in the hands of our youth and young adults that are in the workforce, and they deserve to be able to grow there careers and start a family in the place that they have called home.

I believe in this work, and I know that advocating for our youth and young adults will help the Cape and Islands flourish.

What part of the Cape did you grow up in?

Yarmouth

What is your favorite childhood memory of growing up on Cape?
Growing up on Cape Cod I spent a lot of time with my brother and our friends in the neighborhood at Indian memorial pond in Yarmouth. On hot summer days we would play basketball all day and then cool off in the pond. I've always felt very fortunate to grow up in a place that has plenty of great ponds and beaches. 

What are some activities you enjoy going to in your community?


My favorite hobby would be playing pickup hockey games at Tony Kent arena in Dennis as well as Charles Moore arena in Orleans. Having access to both rinks and being able to play hockey on a weekly basis has been very beneficial to my own health and wellness. I also really enjoy going to Taylor Bray farm in Dennis to see the highland cows and sheep with my family.
