Program Overview
Trabajando en asociación con el condado de Barnstable, HPC brinda administración de casos para jóvenes y adultos jóvenes menores de 25 años que viven solos y tienen dificultades para encontrar o mantener una vivienda. Si es un adulto joven que necesita apoyo, llame o envíe un mensaje de texto a Mary, nuestra administradora de casos de jóvenes y adultos jóvenes, al 774-801-9502.
El programa se centra en adultos jóvenes que corren el riesgo de quedarse sin hogar, incluidos jóvenes que hacen surf en sofás, acampan o viven en automóviles.
The range of services provided includes:
Community Outreach
The Outreach team will be available to connect with young people at various locations, including but not limited to schools, community centers, neighborhoods, partner agency sites and local community events.
Basic
Needs Assistance
Outreach workers will strive to meet your basic needs, including food, clothing, hygiene products, transportation, and more.
Referral Services
Referrals for essential resources such as housing, healthcare, education, employment, parenting support, and more will be provided, as needed.
1:1 Support
Our team is trained to provide individualized support to meet client needs. Outreach workers are committed to remaining flexible and responsive while working with a young adult to achieve stability.

The work we do at HPC is especially important considering the level at which Cape Cod has been gentrified. Average, working people, whether
they came from somewhere else or have always lived here, are just barely getting by.
And young people who are just graduating college, high school or simply trying to build a life for themselves on Cape do not have clear and straight path before them.
It is mine and HPC’s purpose to provide community, support and guidance to all of those who seek our services, and to help them navigate a brutal economy and housing market.
What part of the Cape did you grow up in?
I grew up all over the Lower Cape; Orleans, Brewster, Eastham, Wellfleet.
What is your favorite childhood memory of growing up on Cape?
Going to Bayside with my family in the summertime. Walking out at low tide and finding hermit crabs, moon snails, and the like.
What are some activities you enjoy going to in your community?
I enjoy seeing local theatre productions at the Academy Playhouse and Cape Rep Theater.
What are your favorite thing/interests?
I love to read. Sitting in the Sparrow or Parish park with a good book. I also love to sing and studied Musical Theatre in college

There is a unique gift in being able to help YYA as a young adult myself. I am able to uplift my generation and community who grew up in the same environment and contextual timeframe I have.
On Cape, we have never survived by trying to outpace each other, but by working together to help each other thrive. I am so grateful and privileged that I can contribute to that idea through my position as a Youth Outreach Worker.

What is your favorite childhood memory of growing up on Cape?
My favorite childhood memory of growing up on Cape was during Covid. It was when the first round of restrictions were raised in summer of 2020 and my friends wanted to hang out for the first time in person since the lockdown. I lived on 6A and they on route 28, and none of us could drive yet, so we biked around 15 miles on the rail trail that day just to hang out and explore.
What are some activities you enjoy going to in your community?
Currently I love going to Nauset Fit Co. to work out and arguably more importantly go into the sauna. On the weekends I usually hang around with my friends, swimming in ponds around the Brewster/ Harwich area and having late night fires where we may play music, cook a lot of food, or both. When the weather cools, fishing turns into ice fishing, gardening into sledding, and cookouts into Entertainment Cinemas budget Tuesday movie nights. But no matter the season, I always find time to put my air pods in and take a walk on the hiking trails Cape has to offer.
